NHTSA has opened Audit Query AQ26003 to evaluate whether Avride Inc. submitted complete and accurate crash reports under Standing General Order 2021-01, citing six incident reports in which police records indicate potential injuries the company’s filings did not reflect. The agency’s Office of Defects Investigation opened the query this week after obtaining police reports for crashes Avride reported under the Standing General Order, including crashes for which Avride had not listed a police report as an available data source. NHTSA describes the Audit Query as a standard process for reviewing compliance with legal requirements.

Why Is NHTSA Auditing Avride’s SGO Reports?
ODI identified several incident reports submitted by Avride under the SGO with potentially incomplete or inaccurate information. Certain of those reports indicated that, at the time of submission, Avride was not aware of any injuries resulting from the crashes. Some of the same reports stated that police reports of the crashes were available.
ODI obtained copies of those police reports. It also obtained police reports for other crashes Avride reported under the SGO for which the company did not list a police report as an available source of data. According to NHTSA, the police reports indicate potential injuries were sustained by occupants of Avride vehicles and/or occupants of other vehicles involved in the crashes.
Separately, ODI identified SGO reports stating that an injury was reported or alleged, where subsequent updated reports indicated no injuries were alleged.
What the Audit Query Will Assess
The Standing General Order requires manufacturers and operators named in the order to report certain crashes involving vehicles equipped with automated driving systems or SAE Level 2 driver assistance systems, and to submit an updated report when new information becomes available. NHTSA has previously used enforcement action to address incomplete SGO crash reporting.
NHTSA opened the Audit Query to evaluate reporting accuracy and completeness. As part of the review, the agency will assess whether any reports of prior incidents remain outstanding and whether the reports Avride submitted reflect complete and accurate data based on the information available to the company.
